Penguin II is an instrument for measuring implant stability and osseointegration. Penguin provides accurate and objective measurements of implant stability, and in turn, osseointegration, serving as reliable support when making the decision on whether or not to load.
Penguin II measures the resonance frequency of a MulTipeg and presents it as an ISQ value (ISQ: Implant Stability Quotient). The ISQ value, 1-99, reflects the stability of the implant – the higher the value, the more stable the implant.
The MulTipeg is made from titanium and has an integrated grip for the MulTipeg Driver on top. There are different MulTipegs available made to fit different implant systems and types. To stimulate the MulTipeg into vibration, short magnetic pulses are sent from the instrument tip. The magnetic pulses interact with the magnet inside the MulTipeg and cause the MulTipeg to vibrate. The instrument picks up the alternating magnetic field from the vibrating magnet, calculates the frequency and from that, the ISQ value.
The instrument measures the ISQ-value with a precision of +/- 1 ISQ unit. When mounted onto an implant, the MulTipeg resonance frequency can vary up to 2 ISQ units depending on the tightening torque. The Bluetooth functionality enables the instrument to connect to another Bluetooth device.
The ISQ-scale and clinical guidelines
Values above ISQ 70 indicate a very stable implant with low micro-mobility. This value is typically recommended for one-stage and immediate loading. A second measurement is recommended before the final restoration to verify osseointegration.
Values in the range of 75 ISQ mean the implant is already so stable that osseointegration cannot add stability in a significant way. The proof of osseointegration is the lack of a drop in ISQ. If the implant has a low initial ISQ, osseointegration will add stability over time.
