Quadrant Protocol: A Four-Component, Biology-First Method for Predictable Peri-Implantitis Resolution

Overview

Peri-implantitis requires more than symptom control. The Quadrant Protocol restores a stable host–implant interface by aligning surgical mechanics with tissue biology and surface chemistry. Its four interdependent components—Implant Renewal, Bone Regeneration Induction, Bacteria-Resistant Surface creation, and Enhancement of Osseointegration Surface Properties—are executed to decontaminate, re-center geometry, and rebuild the titanium passivation layer essential for durable health.

1) Implant Renewal (not implantoplasty)

Renewal removes contaminated outer layers while recalibrating the bone-to-implant volume relationship. Unlike thread smoothing, it recenters geometry and eliminates plaque-retentive defects. Critically, it exposes a clean titanium substrate that can repassivate, setting the stage for a stable oxide film and improved soft-tissue adaptation.

2) Bone Regeneration Induction

Targeted debridement plus controlled micro-wounding converts a chronically inflamed site into a vascular, pro-healing bed. By excising non-viable bone and stimulating angiogenesis, the protocol primes predictable new bone formation and graft integration when indicated.

3) Bacteria-Resistant Surface & Passivation Layer

Long-term stability demands a surface that discourages biofilm maturation. The protocol emphasizes surface cleanliness, energy, and micro-/nano-profile to reduce microbial affinity and support a tight mucosal seal. Central to this step is re-establishing the native titanium dioxide passivation layer, which enhances corrosion resistance, lowers ionic release, and improves wettability—factors that collectively diminish bacterial adhesion and reinfection risk.

4) Enhanced Osseointegration Properties

Optimizing wettability and architecture promotes rapid fibrin adhesion, protein adsorption, and osteoblast activity. A reformed passivation layer supports early bone-implant contact, reduces micromotion, and sustains functional loading.

Workflow, Candidates, and Limits

Access, Renewal, Regeneration Induction, Surface/Passivation optimization, and stable closure form a reproducible sequence. Candidates include salvageable implants with adequate bony potential and manageable risks; severe mobility or uncorrectable prosthetics may warrant explantation. Success depends on precise execution and strict maintenance (hygiene reinforcement, periodic debridement, radiographic review).
